I am developing a macro that might add and delete a large number of entities. When this occurs, does this activity enlarge the file excessively, leaving large holes in the database where entities were deleted, and increasing the length by adding new ones at the end, or does Datacad automatically compact the database when closing the file (or at other times?).

I have the option of taking entities that are going to be deleted and redefining them (only if they are the same type) and updating them, or deleting them and creating new ones. Does either option result in a tidier solution?

By default DataCAD will purge the file when it is closed. The bloat scenario would only be an issue during the current session.
