I haven't been able to pinpoint when or why this happens. But, some times all the entities of one layer are grouped. What am I doing? 
This usually happens when areas of a drawing are copied to another area. All the entities copied from each layer were then grouped.
In the new 11.08 there is now a toggle that allows you to retain the original groupings when copying entities.
